This worksheet guides your disciple to set intentional, Spirit-led SMART goals in five important areas of life: Physical, Mental/Emotional, Relational, Spiritual, and Work/School. Using the SMART framework (Specific, Measurable, Achievable, Relevant, Time-bound), it helps create clear, actionable steps for growth.

It also prompts you to identify five people God has placed in your everyday path—those you can pray for regularly and intentionally share God’s love and truth with.

Additionally, there is space for long-term prayer requests and accountability goals to encourage ongoing support and growth.
